1. To participate in the Slim·Fast® Style Your Slim™ Fashion Rewards Card* Rebate (the "Promotion"), purchase multi-packs of Slim·Fast® Meal On-The-Go™ Shakes and Bars, Snack Bars and/or Snack Chews beginning January 1, 2008 through June 30, 2008, cut out and collect the original UPC symbols from the outer packaging and submit with the original cash register receipts and the Slim·Fast® Style Your Slim™ Fashion Rewards Official Mail-In Certificate, (the "Certificate"). The Certificate must be obtained directly from the Slim·Fast® website located at www.Slim-Fast.com, (the "Website") following online registration to become a Slim·Fast® Fashion Rewards member. Registration for the Promotion is free and required to obtain the Certificate. By registering, you will automatically receive two (2) Bonus Points towards your collection of UPCs. Outlined in the chart below is: (1) the total points received for each level of UPCs submitted; and (2) the Slim·Fast Fashion Rewards Card ("Rewards Card") awarded for the total amount of points submitted. This Promotion applies to multi-packs of Slim·Fast® Meal On-The-Go™ Shakes and Bars, Snack Bars and Snack Chews, and is not valid on singles or powders.

2. On or before July 31, 2008, send your rewards card claim submission ("Rewards Card Claim") with all of the following: (a) the completed Certificate obtained online including valid name, U.S. street address, e-mail address, date of birth and the number of points you have accumulated through collecting UPC symbols; (b) the original UPC symbols from multi-packs of Slim·Fast® Ready-to-Drink Shakes and Bars, Snack Bars and/or Snack Chews; (c) the original receipts (dated January 1, 2008 - June 30, 2008) with Slim·Fast® purchases circled, to:

3.Comply with all other terms and conditions set forth below.

LIMIT ONE GIFT CARD CLAIM PER HOUSEHOLD ADDRESS. Duplicate Rewards Card Claims will not be honored or acknowledged. Promotion valid only in the 50 U.S. or D.C.; must be a legal resident of the 50 U.S. or D.C., 18 years of age or older. Purchase any multi-packs of Slim·Fast® Meal On-The-Go™ Shakes and Bars, Snack Bars and/or Snack Chews beginning January 1, 2008 through June 30, 2008. Products purchased before or after these dates are not eligible for the Promotion. Purchase date is determined by date printed on sales receipt. All Rewards Card Claims must be postmarked by July 31, 2008 and received by Slim·Fast®’s fulfillment agency no later than August 7, 2008. Rewards Card Claims from Post Office Boxes, commercial addresses, containing invalid, undeliverable or non-U.S. mailing addresses, incomplete or illegible or any Rewards Card Claim deemed invalid, based on these Terms & Conditions will be rejected and will not be honored, acknowledged or returned. Slim·Fast® is not responsible for printing errors or for lost, late, mutilated, misdirected or postage due mail. Rewards Card Claims must include all components listed in Section 2 above. Rewards Card rights cannot be assigned or transferred and the Promotion is void where taxed, restricted or prohibited by law. No substitutes or rainchecks. Keep copies of all materials submitted because Rewards Card Claims become property of Slim·Fast® and will not be returned. Fraudulent Rewards Card Claims and/or use of multiple addresses to obtain multiple Rewards Cards could result in federal prosecution under mail fraud statutes (Title 18, USC Sections 1341 & 1342). Slim·Fast® reserves the right to substantiate Rewards Card Claims and request additional identification and/or documentation. Rewards Cards will be mailed within four (4) to six (6) weeks from the date of receipt. A third-party rebate fulfillment company processes Rewards Card Claims. Only actual purchasers of the product may participate in the Promotion. Promotion not valid for groups, clubs or organizations, employees of Conopco, Inc., d/b/a Unilever or the fulfillment company or their respective suppliers and/or distributors, and their respective employees. The Promotion is governed by the laws of the State of New Jersey for the resolution of all claims and disputes. If any part of these terms and conditions is deemed to be invalid or otherwise unenforceable or illegal, the balance of the terms and conditions shall remain in effect and shall be construed in accordance with its terms as if the invalid or illegal provision were not contained herein.
