You qualify by mailing in the required number of UPC symbols from the cardboard outer packaging of multi-pack Slim·Fast® Meal On-The-Go™ Shakes and Bars, Snack Bars and/or Snack Chews (30-count Shake Club packs will have the UPC symbol on plastic wrap), the corresponding original cash register receipts dated January 1, 2008 - June 30, 2008 and the mail-in certificate form. No, only multi-packs of Slim·Fast® Meal On-The-Go™ Shakes and Bars, Snack Bars and/or Snack Chews qualify for the program. Individually sold single shakes or bars do not qualify for the Fashion Rewards Card Program. You may only submit one claim for a maximum rewards card value of $40.00 (requires 23 UPC's + 2 bonus points). You have until July 31, 2008 to mail-in your UPC's with original cash register receipts (dated between January 1, 2008 - June 30, 2008) plus mail-in certificate, and must be postmarked no later than August 7, 2008. Your Fashion Rewards Card comes with lost/stolen protection which protects you from fraudulent use of a lost or stolen card with no penalty to you. Upon discovering the loss, you would immediately notify American Express®, who will issue a replacement card containing the remaining balance at the time the card was reported missing.
